Consent to Treatment and Other Acknowledgements
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
What to Expect on Your First Visit
At Atlas Total Health, we believe your path to better health starts with a warm welcome and a personalized approach. Your first visit is all about getting to know you, understanding your health concerns, and beginning your journey to feeling your best.
Here's what you can expect during your first appointment:

1. A Warm Welcome
As you enter our office, you'll be greeted with a smile—and by name, whenever possible. We're here to make you feel comfortable from the start.

2. Check-In and Paperwork
Our team will introduce themselves and guide you through the check-in process. Someone will ask for your photo ID and insurance card, then walk you through the intake paperwork so you know exactly what you're filling out and why.

3. Office Tour
Once your paperwork is complete, you will be given a quick tour of our office so you feel right at home. It's then time to head to the exam room to begin your initial assessment.

4. Vitals and Health History
Either one of our Chiropractic Therapy Assistants or the Doctor will take your vitals and ask a few basic questions about your health history and current concerns.

5. Chiropractic Examination
Next, the Doctor will perform a thorough exam to better understand your spine, posture, and any areas of concern. This helps us pinpoint what's going on and how we can best help you.

6. X-Rays (if needed)
Depending on your specific symptoms and history, X-rays may be needed to get a clearer view of your spine and joints.

7. Therapeutic Treatments
Before you leave, you may experience some initial therapy to help you start feeling relief. This could include:
- Electrical Stimulation (E-Stim)
- Traction
- Gentle stretching
These therapies help relax your muscles and prepare your body for care.

8. Your Next Visit: Report of Findings
Before you go, we'll schedule a follow-up appointment within 48 hours (or as soon as possible). During this visit, the Doctor will:
- Go over the results from your initial exam and any significant X-ray findings
- Review a custom care plan tailored to your needs
- Begin treatment based on the care plan
